PATCH is a health and wellness clinic held during the school day when middle and high school students whose parents have given consent may be seen by a doctor. The services at the clinic can supplement, but are not meant to replace care given by the student's regular medical provider.
Services include sports and camp physicals, immunizations, sick visits, smoking reduction/cessation guidance, injury assessments, health education, and prevention and risk reduction activities.
PATCH is held once a week at Pittsfield Middle High School.
The doctors are residents from the NH/Dartmouth Family Practice Residency Program at Concord Hospital, supervised by Dr. Robert Zahn and Dr. Suzanne Boulter.
Any Pittsfield Middle High Student whose parent has signed a consent form may be seen at PATCH. Parents can request a consent form by calling the school (435-6701). Students who are 18 years old may legally consent to receive services at PATCH.
For students who have no regular medical provider, PATCH can help find a provider in the community or in Concord. Also, if qualified students (or any siblings under 19 years of age) have no health insurance, PATCH staff can help their family's apply to the NH Healthy Kids Insurance Program.
